Computer Applications Experiment 1

 In this experiment, a questionnaire was designed using a word processing package to collect information about a particular disease. Different types of questions related to symptoms, causes, prevention, treatment, and patient awareness were included in a clear and organized format. Various formatting tools such as tables, fonts, alignment, and bullet points were used to make the questionnaire attractive and easy to understand. This experiment helped in developing skills in document creation, formatting, and preparing health-related survey forms for data collection.
